 Online registration is mandatory for all PAHA players!

 MISC. REGISTRATION INFORMATION

1. Follow the registration steps above, registering first with USA Hockey, and then with PAHA.

2. Fees are as follows and are in addition to the USA Hockey fee:
Bantams: $425
Peewees/12U: $375
Squirts/10U: $260
Mite 2: $145
Mite 1: $125

3. The registration deadline is September 30, 2011. You are encouraged to register early to take advantage of payment plans and early updates on the upcoming season. There will be a $75 late fee charged to returning players that do not register on time.

4. The fundraising fee is $200 per family. This means that each family will be required to raise $200 in profit for the association. Families may opt out of the fundraiser by paying $200 to PAHA. Fundraising information will be available on the website by Oct. 1, 2011. If you wish to pay your fundraising money early, please contact Ryan to make arrangements.

5. First year players do not pay registration fees, but they still must participate in the fundraiser as described in #4.

6. Please refer to the online schedule available on the website for information about ice time, practices, camps and tryout dates. For new families - check for slots labeled at the level your child is registered - Mites, Squirts, etc.

7. You will receive a confirmation e-mail after you have completed registration. If you don't receive a confirmation e-mail, please contact Ryan Krajewski. Please review your confirmation e-mail carefully.

8. Shortly after you have registered, you will receive a Consent to Treat form in your e-mail. Please either fill it out, save it, and e-mail it back to Ryan; or print it, fill it out, and submit to Ryan's box in the concession stand. Your child is not allowed on the ice until this form has been submitted. If you did not receive the form, you may download it here: Consent to Treat

9. Birth Certificates: Good news! This is the last year that Minnesota Hockey is going to require a birth certificate for returning players. This means that birth certificates will be required for new players only, including first year players. In the past, we only required birth certificates from players Squirt/10U age and older. If your player has not previously submitted a birth certificate, I will be requesting one from you by e-mail. A copy/scan/photo of a government issued birth record is required if you have not previously submitted one. Players will be ineligible for game competition unless a birth certificate is submitted. A comprehensive list of all players that have not submitted a birth certificate will be posted online and at the rink on October 5th, and players will have until October 24th to submit a copy either by e-mail or to Ryan's box in the concession. Failure to comply will result in ineligible/uninsured status with USA Hockey and PAHA.
